How would you rate the nausea: mild, moderate, or severe?  
How many episodes of vomiting?  
Other side effects of treatment? (diarrhea, constipation, dry mouth, fatigue, other)  
List medicines, vitamins, or herbal therapies you took to treat or prevent your nausea and vomiting today.
